ABSTRACT

Introduction Mucoepidermoid carcinoma (MEC) is a tumor originated from the epitheliumof the glandular excretory ducts and has highly variable biological potential. It is the most prevalent cancer of the salivary glands. The present report aims to describe a case of nasal mucoepidermoid carcinoma that developed after adjuvant radiotherapy (RT) treatment of a recurrent pituitary macroadenoma. Case Report Male patient, 62 years old, presented with recurrent nasal epistaxis on the right, associated with intense pulsatile headache, visual analogical scale (VAS) 10/10, with improvement only with the use of opioids andmorphine. After undergoing oncological screening and study by imaging exams, the presence of an expansive seal lesion with suprasellar extension was seen, involving the medial wall of the cavernous segment of the right carotid artery and the anterior cerebral artery, as well as the presence of a new expansive lesion in the right nasal cavity, with ethmoid bone invasion superiorly and medial orbit wall invasion laterally, compressing the ipsilateral optic nerve canal. Discussion Sinonasal neoplasms represent a small portion of all malignancies of the upper aerodigestive tract, accounting for<5% of these neoplasms. The development of MEC involves risk factors such as occupational issues, history of trauma and surgery involving the nasal area, and radiation exposure, as in previous RT. Conclusion Mucoepidermoid carcinoma is an uncommon neoplasia and can be associated with RT treatment, as used in cases of recurrent pituitary macroadenoma. In general, surgical resection to obtain free margins of neoplastic tissue is the aimed treatment, seeking better prognosis.

ABSTRACT

OBJECTIVE@#To investigate the clinicopathological characteristics of micro and mini parotid gland tumors and to provide reference for their clinical diagnosis and treatment.@*METHODS@#Patients with parotid gland tumors treated in the Department of Oral and Maxillofacial Surgery, Peking University School and Hospital of Stomatology from December 2012 to April 2020 were selected. Relevant clinical data of the patients with tumor diameter ≤20 mm detected by preoperative CT were collected to analyze the clinicopathological characteristics and prognosis of micro and mini parotid gland tumors. And the collected data were divided into two groups with diameter 11-20 mm and diameter ≤10 mm according to tumor diameter measured by preoperative CT. The clinicopathological differences between the two groups were statistically analyzed.@*RESULTS@#A total of 2 067 patients with primary epithelial parotid gland tumors were collected, and 685 patients with tumor diameter ≤20 mm were examined by CT, accounting for 33.1%. The ratio of male to female patients with micro and mini parotid gland tumors was 1 ∶1.93, the average age was (45.3±13.8) years (12-83 years), and the median course of disease was 12 months (1 week to 30 years). Among them, 635 cases (92.7%) were benign tumors, 50 cases (7.3%) were malignant tumors, and the ratio of benign to malignant was 12.7 ∶1. The most common benign tumor was pleomorphic adenoma, and the most common malignant tumor was mucoepidermoid carcinoma. The micro and mini parotid gland tumors were divided into 11-20 mm group (n=611) and ≤10 mm group (n=74), the clinical characteristics comparison of the two groups of gender ratio, average age, course of di-sease had no statistical difference (P>0.05). In the 11-20 mm diameter group, the percentage of benign and malignant tumor was 92.8% (567/611) and 7.2% (44/611) respectively, and the ratio of benign to malignant tumors was 12.9 ∶1. In the ≤10 mm diameter group, the percentage of benign and malignant tumor was 91.9% (68/74) and 8.1% (6/74) respectively, and the ratio of benign to malignant tumors was 11.3 ∶1. There was no significant difference between the two groups (P>0.05). Fifty patients with malignant tumor were followed up for the median follow-up period of 39.5 months (1-91 months). Local recurrence occurred in 2 patients with one death. The overall 2-year survival rate was 93.7% and the 5-year survival rate was 89.3%.@*CONCLUSION@#The majority of micro and mini parotid gland tumors was benign lesion. There was a good prognosis for micro and mini parotid gland carcinoma. Early surgical treatment was recommended for micro and mini parotid gland tumors.

ABSTRACT

Mucoepidermoid carcinoma (MEC) is the most common malignant salivary gland tumor. Although the parotid gland is the most common site of involvement, other major salivary glands and the minor salivary glands­most commonly of the palate­also can be involved. The management of mucoepidermoid carcinoma depends on the grade of the tumor and the adequacy of resection. We present the case of a 56-year-old female presenting a painless progressive cheek mass over 2 months. Imaging and fine-needle aspiration cytology provided the diagnosis of Mucoepidermoid carcinoma. A superficial parotidectomy was done, and the histopathology revealed a predominantly cystic tumor with a bilayered epithelium of oncocytic and basal cells. Moderate nuclear pleomorphism with infiltration of atypical squamous cells in few glandular cysts was seen. Special staining revealed the presence of intracellular mucin. A diagnosis of Warthin-like variant of MEC was made, based on these findings. After the surgical procedures, the patient is disease-free at 8 months of follow-up. The Warthin-like variant is a rare variant of MEC with fewer than 10 cases described in the English literature. Various differential diagnoses include the malignant transformation of Wartin tumor (WT), squamous metaplasia of WT, and metastasis from a distant primary. We emphasize the role of routine microscopy in identifying rare variants of common malignancies. Even though translocation studies are helpful in diagnosis, the typical histopathological findings should confirm it.

RESUMEN Introducción: Las neoplasias malignas de glándulas salivales se ubican entre las primeras diez localizaciones de incidencia oncológica. Objetivo: Describir el comportamiento epidemiológico-clínico-terapéutico de los tumores malignos de glándulas salivales en pacientes mayores de 15 años en la provincia de Sancti Spiritus entre 2012 y 2015. Métodos: Se realizó un estudio observacional, descriptivo, transversal. La muestra estuvo constituida por 39 pacientes mayores de 15 años, con neoplasias en glándulas salivales que acudieron al servicio de Cirugía Maxilofacial entre el 1 de enero de 2012 y el 31 de diciembre de 2015. Resultados: Predominó el sexo masculino y el rango de edad comprendido entre 45-64 años. Los municipios con mayor número de pacientes fueron Sancti Spiritus y Cabaiguán. El 23,1 por ciento de los obreros están expuestos a los rayos solares y sustancias químicas. Los parámetros clínicos más frecuentes fueron el aumento de volumen, las molestias, el dolor en la región. La glándula parótida fue el sitio anatómico de aparición más frecuente y el carcinoma mucoepidermoide la más recurrente de las neoplasias malignas de glándulas salivales. La supervivencia global a los dos años fue de 84,1 por ciento. Conclusiones: Las neoplasias malignas de glándulas salivales constituyen un problema de salud en el centro del país. Más de la mitad de los casos se diagnostican en etapas avanzadas debido, en parte, a la procedencia rural (zonas de difícil acceso) de gran parte de la muestra. Las cifras de supervivencia se encuentran acorde a parámetros internacionales(AU)


ABSTRACT Introduction: Malignant neoplasms in the salivary glands are among the first ten locations of cancer incidence. Objective: To describe the epidemiological, clinical and therapeutic behavior of malignant salivary gland tumors in patients older than 15 years in Sancti Spiritus Province between 2012 and 2105. Methods: An observational, descriptive, cross-sectional study was carried out. The sample consisted of 39 patients older than 15 years, with salivary gland neoplasms and who attended the maxillofacial surgery service between January 1st, 2012 and December 31st, 2015. Results: The male sex and the age range between 45-64 years predominated. The municipalities with the highest number of patients were Sancti Spiritus and Cabaiguán. 23.1 percent of workers are exposed to sunlight and chemicals. The most frequent clinical parameters were volume increase, discomfort, localized pain. The parotid gland was the most frequent anatomical onset site and the mucoepidermoid carcinoma was the most recurrent of the malignant neoplasms in the salivary glands. The two-year overall survival was 84.1 percent. Conclusions: Malignant neoplasms of salivary glands constitute a health concern in the central region of the country. More than half of the cases are diagnosed in advanced stages due, in part, to the rural origin (hard-to-reach areas) of much of the sample. Survival rates are coincident with international parameters(AU)

Abstract Introduction: Cell division cycle-7 protein is a serine/threonine kinase that has a basic role in cell cycle regulation and is a potential prognostic or therapeutic target in some human cancers. Objectives: This study investigated the expression of cell division cycle-7 protein in benign and malignant salivary gland tumors and also its correlation with clinicopathologic factors. Methods: Immunohistochemical expression of cell division cycle-7 was evaluated in 46 cases, including 15 adenoid cystic carcinoma, 12 mucoepidermoid carcinoma, 14 pleomorphic adenoma, and 5 normal salivary glands. Cell division cycle-7 expression rate and intensity were compared statistically. Results: The protein was expressed in almost all tumors. The intensity and mean of cell division cycle-7 expression were higher in malignant tumors in comparison with pleomorphic adenomas (p = 0.000). The protein expression was correlated with tumor grades (p = 0.000). Conclusions: The present study demonstrated cell division cycle-7 overexpression in malignant salivary gland tumors in comparison with pleomorphic adenomas, and also a correlation with tumor differentiation. Therefore, this protein might be a potential prognostic and therapeutic target for salivary gland tumors.


Resumo Introdução: A cell division cycle-7 é uma serina/treonina quinase que tem um papel básico na regulação do ciclo celular e é um potencial marcador prognóstico ou terapêutico em alguns tipos de câncer humano. Objetivos: Este estudo investigou a expressão de cell division cycle-7 em tumores de glândulas salivares benignos e malignos e também sua correlação com fatores clínico-patológicos. Método: A expressão imuno-histoquímica de cell division cycle-7 foi avaliada em 46 casos, incluindo 15 carcinomas adenoide císticos, 12 carcinomas mucoepidermoides, 14 adenomas pleomórficos e 5 glândulas salivares normais. A taxa de expressão e a intensidade da proteína cell division cycle-7 foram comparadas estatisticamente. Resultados: A proteína foi expressa em quase todos os tumores. A intensidade e a média da expressão de cell division cycle-7 foram maiores em tumores malignos em comparação com adenoma pleomórfico (p = 0,000). A expressão da proteína foi correlacionada com os graus do tumor (p = 0,000). Conclusões: O presente estudo demonstrou a superexpressão de cell division cycle-7 em tumores malignos de glândulas salivares quando comparada com o adenoma pleomórfico, além de uma correlação com a diferenciação de tumores. Portanto, essa proteína pode ser um potencial marcador prognóstico e terapêutico para tumores de glândulas salivares.

ABSTRACT Objective To evaluate the clinical-pathological profile of patients with minor salivary gland neoplasms. Methods A retrospective study of specific cases diagnosed as benign and malignant tumors of the minor salivary glands was performed. The data were collected from medical records of patients seen at a hospital over a period of 15 years. The sample was made up of 37 cases. For the pathological study, slides containing 5μm thick sections stained with hematoxylin and eosin were used. The data were tabulated using descriptive statistics. Results Malignant neoplasms represented 70.3% of cases. The mucoepidermoid carcinoma was the most common neoplasm (45.9%), followed by pleomorphic adenoma (24.4%). Most patients were female (70.3%), aged between 71 and 80 years. The palate (67.6%) and the retromolar region (10.8%) were the most affected sites. Conclusion Mucoepidermoid carcinoma was the most common tumor in minor salivary glands. These tumors are more common in females aged over 40 years. The palate was the most common affected site.


RESUMO Objetivo Avaliar o perfil clínico-patológico de pacientes com neoplasias de glândula salivar menor. Métodos Foi realizado um estudo retrospectivo de casos específicos diagnosticados como neoplasias benignas ou malignas de glândula salivar menor. Os dados foram coletados dos prontuários dos pacientes atendidos em um hospital no período de 15 anos. A amostra final foi de 37 casos. Para o estudo histopatológico, foram usadas lâminas contendo secções com 5μm de espessura, coradas pela técnica de hematoxilina e eosina. Os dados foram tabulados de forma descritiva. Resultados As neoplasias malignas representaram 70,3% dos casos. O tipo histológico mais prevalente foi o carcinoma mucoepidermoide (45,9%), seguido do adenoma pleomórfico (24,4%). A maioria dos pacientes era do sexo feminino (70,3%), com idade entre 71 e 80 anos. O palato (67,6%) e a região retromolar (10,8%) foram os sítios mais acometidos. Conclusão O carcinoma mucoepidermoide foi o tumor mais comum das glândulas salivares menores. Estes tumores foram mais comuns em mulheres com mais de 40 anos. O palato foi o sítio mais acometido.

As neoplasias de glândulas salivares exibem uma grande diversidade morfológica e comportamentos biológicos variados o que suscita o interesse na pesquisa destas lesões. A disseminação das células tumorais é um passo inicial para a progressão de neoplasias malignas e, dentro deste contexto, os vasos linfáticos neoformados são considerados essenciais para que ocorra essa disseminação. O papel do VEGF (fator de crescimento endotelial vascular) na formação dos vasos é fato conhecido mas, pouco se sabe a respeito de sua participação em tumores de glândula salivar. Desta forma, o objetivo deste estudo foi avaliar a expressão do VEGF-C e VEGF-D, a densidade linfática tumoral (D2-40) e a proliferação endotelial linfática (dupla marcação D2-40/Ki-67) em uma série de neoplasias de glândulas salivares. A amostra foi composta por 20 adenomas pleomórficos, 20 carcinomas adenóides císticos, 20 carcinomas mucoepidermóides e 10 casos de tecido glandular salivar com características de normalidade para efeito comparativo. Todos os casos estudados exibiram expressão positiva para VEGF-C em região peritumoral e intratumoral, não sendo encontrada diferenças de imunoexpressão entre os grupos. No entanto, o grupo dos carcinomas adenóides císticos demonstrou diferença significativa da imunoexpressão do VEGF-C segundo o padrão cribriforme e sólido (p = 0,004). Salivary gland neoplasms exhibit a great morphological diversity and varied biological behavior which raises the interest in the study of these lesions. The spread of tumor cells is an early step in the progression of malignancies and the neoformed lymphatic vessels are considered essential in tumor dissemination. Vascular endotelial growth fator (VEGF) is a family of proteins involved in angiogenesis e lymphangiogenesis. However, in salivar tumors we have limited information on the expression. The aim of this study was to assess the expression of VEGF-C and VEGF-D, lymphatic vessel density (single-staining D2-40) and lymphatic endothelial proliferation (double labeling D2-40/Ki-67) in a series of salivary glands neoplasms. We selected 20 cases of pleomorphic adenoma, 20 of mucoepidermoide carcinoma, 20 of adenoid cystic carcinoma and 10 tissue sample of normal salivary gland. All cases studied showed positive expression of VEGF-C in intratumoral and peritumoral region, no differences in immunoreactivity was found between the groups. However, the group of adenoid cystic carcinoma showed a significant difference in immunoreactivity of VEGF-C by the cribriform and solid pattern (p = 0.004). Most of the cases included in this study showed weak immunoreactivity for VEGF-D in intratumoral and peritumoral region. In the assessment of lymphatic endotelial density peritumoral, intratumoral and total, the groups showed an increasing gradient, with lower values for the group of pleomorphic adenomas followed by mucoepidermoid carcinoma and adenoid cystic carcinoma. Lymphatic endothelial cell density was higher in malignant than benign tumors. No correlation was observed between the immunoreactivity of VEGF-C and VEGF-D in relation to tumor lymphatic density and lymphatic endothelial proliferation.

ABSTRACT

O adenoma pleomórfico (AP), o carcinoma mucoepidermóide (CME) e o carcinoma adenóide cístico (CAC) representam tumores frequentes em glândula salivar. A via de sinalização Sonic Hedgehog (Hh) e o Transdutor de sinal e ativador da transcrição 3 (STAT3) desempenham funções importantes na proliferação celular, favorecendo o desenvolvimento tumoral e a proteína MCM3 tem sido considerada uma nova classe de marcadores de proliferação celular. Portanto, o presente trabalho propõe-se a estudar componentes da via Hh, bem como o STAT3 e o MCM3 em neoplasias de glândula salivar, na tentativa de adicionar informações sobre as características biológicas dessas neoplasias. Foram utilizados 9 casos de AP, 17 casos de CAC e 20 casos de CME e, por meio da técnica imunoistoquímica, realizou-se a detecção das seguintes proteínas: SHH, GLI1, SUFU, HHIP, STAT3 e MCM3. No AP, observou-se alta expressão citoplasmática de SHH e SUFU, e baixa expressão de STAT3 e MCM3. No CAC, observou-se alta expressão de GLI1, HHIP e STAT3 e baixa expressão de SHH, SUFU e MCM3. No CME, observou-se alta expressão de SHH, GLI1, SUFU e HHIP e baixa expressão de STAT3 e MCM3. Quando comparado entre os tipos tumorais, observou-se diferença estatisticamente significante para expressão de SHH (p=0.0064), STAT3 (p=0.0003) e MCM3 (p=0.0257)...


The pleomorphic adenoma (PA), mucoepidermoid carcinoma (MEC) and the adenoid cystic carcinoma (ACC) are common tumors arising from salivary glands. The Sonic Hedgehog signaling pathway (Hh) and signal transducer and activator of transcription 3 (STAT3) play important roles in cell proliferation, favoring tumor growth. The MCM3 protein has been considered as a novel class of cell proliferation markers. The aim of this investigation was to study components of the Hh pathway, as well as STAT3 and MCM3 in salivary gland neoplasms in an attempt to add information about the biological characteristics of these neoplasms. We used 9 cases of PA, 17 cases of ACC and 20 cases of MEC. Using immunohistochemistry, were investigated: SHH, GLI1, Sufu, HHIP, STAT3 and MCM3. In PA, there was high expression of cytoplasmic SHH and Sufu, and low expression of STAT3 and MCM3. In the ACC, there was high expression of GLI1, HHIP and STAT3 and low expression of SHH, SUFU and MCM3. In the MEC, we observed high expression of SHH, GLI1, SUFU and HHIP and low expression of STAT3 and MCM3. ABSTRACT

OBJECTIVE: The maximum standardized uptake value (SUVmax) of pulmonary mucoepidermoid carcinoma (PMEC) in fluorine-18fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) was evaluated as a preoperative predictor of pathologic grade and survival rate. MATERIALS AND METHODS: Twenty-three patients who underwent preoperative PET/CT and complete resection for PMEC were enrolled. The optimal cut-off SUVmax for tumor grade was calculated as 6.5 by receiver operating characteristic curve. The patients were divided into a high SUV group (n = 7) and a low SUV group (n = 16). Clinicopathologic features were compared between the groups by chi2 test and overall survival was determined by Kaplan-Meier analysis. RESULTS: The mean SUVmax was 15.4 +/- 11.5 in the high SUV group and 3.9 +/- 1.3 in the low SUV group. All patients except one from the low SUV group had low grade tumors and all had no nodal metastasis. The sensitivity and specificity of SUVmax from PET/CT for predicting tumor grade was 85.7% and 93.8%, respectively. During the follow-up period (mean, 48.6 +/- 38.7 months), four patients from the high SUV group experienced cancer recurrence, and one died of cancer. In contrast, none of the low SUV group had recurrence or mortality. Five-year overall survival rate was significantly higher in the low SUV group (100% vs. 71.4%, p = 0.031). CONCLUSION: Pulmonary mucoepidermoid carcinoma patients with high SUVmax in PET/CT had higher tumor grade, more frequent lymph node metastasis and worse long-term outcome. Therefore, PMEC patients with high uptake on PET/CT imaging might require aggressive mediastinal lymph node dissection and adjuvant therapies.

ABSTRACT

The most common types of salivary gland tumors are the pleomorphic adenoma and the mucoepidermoid carcinoma (MEC). MEC mainly occurs in the parotid gland; intraorally MEC appears as asymptomatic swellings in minor salivary glands, most commonly occurs on palate. It shows a variety of biological behaviors and variable natural history. Clinical, radiological and histopathological findings of palatal MEC in a young woman presented here. Surgical resection was done. Follow-up of one and half year did not show any recurrence.

ABSTRACT

Warthin's tumor also known as papillary cystadenoma lymphomatosum is a common benign salivary gland neoplasm that occurs exclusively in parotid gland. Rarely, the tumor can undergo carcinomatous or lymphomatous transformation of epithelial or lymphoid component, respectively. Herein, we describe a case of 55-year-old female who had undergone parotidectomy for a rapidly growing tumor in the right parotid gland. The case was diagnosed as mucoepidermoid carcinoma developed in a setting of Warthin's tumor based on the histomorphology, special staining characters, and immunohistochemical findings. The pathogenesis and differential diagnoses of such rare malignancy has been discussed briefly.

ABSTRACT

Los tumores de las glándulas salivales representan entre el 2 y 3 por ciento de las neoplasias de cabeza y cuello. El carcinoma mucoepidermoide (CME) es el tumor maligno más común en las glándulas salivales; su comportamiento biológico se relaciona con el grado histológico tumoral (bajo, intermedio y alto). Está formado por células mucosas, epidermoides, intermedias, columnares y claras. El objetivo del presente trabajo fue presentar la casuística de CME de nuestro laboratorio en el período 1999-2009 para determinar grado histológico y potencial proliferativo como factores pronósticos ya que éstos son importantes para guiar el tratamiento y diseñar recomendaciones para los pacientes. Cortes seriados de 13 casos de CME se colorearon con H/E, Masson, PAS, Dane/Alcian blue y Azul de toluidina. Se realizó inmunomarcación para ki67. Se analizó su grado histológico según el sistema de gradación tumoral de Auclair et al., que evalúa variables observables con microscopio óptico. 8 tumores se localizaron en parótida, 3 en paladar, 1 en submandibular y 1 en glándulas genianas. Histológicamente 3 casos fueron de alto grado, 2 de grado intermedio y 8 de bajo grado, según sus características citológicas, patrón de invasión y tipo celular. En dos casos de bajo grado se observaron cristales de colesterol, macrófagos y células gigantes de cuerpo extraño. La expresión de ki67 fue importante en tumores de alto grado. La parótida fue la más afectada seguida por paladar y submaxilar, lo que acuerda con la bibliografía. El grado histológico tumoral se asoció con la expresión de Ki 67, factores pronósticos relevantes en el CME, que deben asociarse a estudios cínicos y análisis de supervivencia. Continuaremos el presente trabajo con la realización de estudios de los residuos azúcares terminales y subterminales de los carbohidratos con lectinhistoquímica, para detectar modificaciones sutiles de la glicosilación de las proteínas celulares, rasgo común en el proceso de transformación neoplásica.


Tumors of the salivary glands represent 2-3 percent of head and neck neoplasms. Mucoepidermoid carcinoma (MEC) is the most common epithelial neoplasm in the salivary glands; the biological behavior relates to a histological grade of the tumor (low, intermediate and high grade). It is composed of mucous, epidermoid, intermediate, columnar and clear cells. We undertook this study to analyze the histological grade and proliferative potential as prognostic factors, important to guide the treatment and design recommendations, in a series of 13 patients who were treated for MEC of principal and accessory salivary glands between 1999 and 2009. Serial cuts of 13 cases of MEC were stained using H/E, Masson, PAS, Dane/Alcian blue and Toluidine blue. Inmunostaining for ki67 were made. Histologically, the tumors were classified as low, intermediate and high grade, according to Auclair et al. system. 8 tumors were located in the parotid, 3 in the palate, 1 in submaxillary and 1 in the genian glands. 3 cases were of high grade, 2 of intermediate grade and 8 of low grade, according to its cytological characteristics, invasive pattern and cell type. In 2 cases of low grade cholesterol clefts, macrophages and foreign-body giant cells were observed. The ki67 expression was important in high grade tumors. The parotid was the most affected gland, followed by the palate and sub maxillary, which agree with the bibliography. The tumoral histological grade was associated with the ki67 expression, relevant prognosis factors in MEC. However, its prognostic value should be validated with clinical studies and survival analyses. Complementary studies to compare carbohydrates of the normal salivary glands must be carried out because subtle alterations in tumor cells glycosylation can influence their biological behavior.

La producción de saliva es una de las principales funciones que cumplen los órganos presentes en la cavidad bucal, esta labor la desarrollan las glándulas salivales, tanto mayores como menores. Estas últimas se encuentran distribuidas en diversas zonas de la cavidad bucal, como lengua, labios, paladar y carrillos. En la lengua pueden encontrase las glándulas de Blandin y Nuhn, Weber y Von Ebner. Dichas glándulas pueden ser asiento de diversas patologías tanto benignas como malignas, el objetivo de esta revisión bibliográfica es dar a conocer las distintas alteraciones que se pueden presentar, sus características y tratamiento


The production of saliva is the principal functions that fulfill the present organs in the oral cavity, the salivary glands develop this labor, as major as minors. The above mentioned are distributed in diverse zones of the oral cavity, like tongue, lips, palate and jowl. In the tongue they can be finding the glands of Blandin and Nuhn, Weber and Von Ebner. The above mentioned glands can be a seat of diverse both benign and malignant pathologies, the aim of this bibliographic revision is to announce the different alterations that they can present, her characteristics and treatment

PURPOSE: To describe and analyze the features of a cases series of patients with primary epithelial neoplasms of the lacrimal gland, its surgical treatment, and histopathological findings. METHODS: Retrospective evaluation of files from patients with primary epithelial neoplasms of the lacrimal gland in the period from 1997 to 2007. All patients with primary epithelial tumors of the lacrimal gland were included in this study. Data on gender, age, clinical features, surgical treatment, histopathological findings and follow-up were collected. The slides with histological sections of the tumors were reviewed by the same pathologist. RESULTS: During the study period, there were 12 patients, 5 (41.7 percent) with benign tumors, all pleomorphic adenomas (benign mixed tumor) and 7 (58.3 percent) with malignant neoplasms, thus distributed: four cases of adenoid cystic carcinoma, two of mucoepidermoid carcinoma and one carcinoma expleomorphic adenoma. Globally, patients mean age was 54.1 years-old (ranging from 14 to 70 years-old), with mean age of 52.4 years-old (ranging from 14 to 65 years-old) for benign neoplasms, and 55.3years-old for malignant neoplasms (ranging from 26 to 70 years-old). Clinical follow-up information, ranging from 2 to 10 years-old, was available for all patients. Three patients developed distant metastasis and died of disease. CONCLUSIONS: The most frequent primary epithelial neoplasms of the lacrimal gland were pleomorphic adenoma and adenoid cystic carcinoma during the study period. Malignant tumors were more frequent than benign tumors. The histopathological diagnosis and the disease initial stage can play a significant role in patient's survival.


OBJETIVO: Descrever e analisar as características de uma série de casos de portadores de neoplasias epiteliais primárias da glândula lacrimal, o tratamento cirúrgico, assim como os achados histopatológicos. MÉTODOS: Avaliação retrospectiva dos arquivos de pacientes com neoplasias epiteliais primárias da glândula lacrimal, no período de 1997 até 2007. Todos os pacientes com tumores epiteliais primários da glândula lacrimal foram incluídos neste estudo. Foram analisados os dados sobre sexo, idade, características clínicas, tratamento cirúrgico, achados histopatológicos e seguimento dos pacientes. As lâminas com secções histológicas dos tumores foram revisadas pelo mesmo patologista. RESULTADOS: No período do estudo, foram encontrados 12 pacientes, sendo 5 (41,7 por cento) portadores de tumores benignos, todos adenomas pleomórficos (tumor benigno misto), e 7 (58,3 por cento) com neoplasias malignas, assim distribuídos: quatro casos de carcinoma adenóide cístico, dois de carcinoma mucoepidermóide e um de carcinoma ex-adenoma pleomórfico. Analisando-se de modo global, a idade média dos portadores foi de 54,1 anos (variando de 14 a 70 anos); com média de idade de 52,4 anos (variando de 14 a 65 anos) para neoplasias benignas, e 55,3 para neoplasias malignas (variando de 26 a 70 anos). Informações do seguimento, variando de 2 a 10 anos, estavam disponíveis para todos os pacientes. Três pacientes desenvolveram metástases distantes e morreram devido à doença. CONCLUSÕES: A maioria das neoplasias epiteliais primárias da glândula lacrimal foi o adenoma pleomórfico e o carcinoma adenóide cístico no período de estudo. Os tumores malignos foram mais frequentes que os benignos. O diagnóstico histopatológico e o estadiamento inicial da doença podem desempenhar uma papel significante na sobrevida do paciente.

ABSTRACT

The association among clinicopathological features and c-erbB-2 oncoprotein expression was evaluated in twenty-nine cases of intra-oral mucoepidermoid carcinoma (MEC). MEC was prevalent in the female gender (79.3 percent), tumors were more frequent in ages between 21 and 40 years (48.3 percent), and the palate was the most commonly affected site (72.4 percent). Microscopically, 27 cases (93.1 percent) were classified as low grade of malignancy. The c-erbB-2 expression was considered positive in 9 (31 percent) cases and no significant association (p>0.05) was found among protein expression and gender nor between patient age and site or histological grade of the lesion. c-erbB-2 expression in MEC may reflect intrisinc biologic properties of salivary gland neoplasms and may be linked to histogenesis and cellular differentiaton.


Fueron evaluados 29 casos de carcinoma mucoepidermoide intraoral en sus aspectos clínico-patológicos, además de la expresión de la oncoproteina c-erbB-2. El carcinoma mucoepidermoide fue predominante en las mujeres (79,3 por ciento), siendo más frecuente en individuos entre 21 y 40 años de edad (48,3 por ciento). El paladar fue el sitio más comunmente afectado (72,4 por ciento). Microscópicamente, 27 casos (93,1 por ciento) fueron clasificados como de baja malignidad. La expresión del c-erbB-2 se consideró positiva en 9 (31 por ciento) casos y no fue observada ninguna asociación significativa (p>0,05) entre la expresión de la proteína y género, ni entre la edad de los pacientes y el sitio o el grado histológico de la lesión. La expresión de la c-erbB-2 en el carcinoma mucoepidermoide puede mostrar las propiedades biológicas intrísecas de las neoplasias de las glándulas salivales.

ABSTRACT

Os autores relatam o caso de um paciente de 13 anos com tosse crônica e pneumonia recorrente, que revelou, na broncoscopia, massa vegetante obstruindo brônquio segmentar superior direito. Foi realizada biópsia, a qual revelou carcinoma mucoepidermóide de baixo grau. O paciente foi submetido posteriormente a bilobectomia. Atualmente encontra-se bem, em seguimento de 14 meses em nosso serviço.


The authors report a case of patient of 13 years of age that presented chronic cough and recurrent pneumonia, whose bronchoscopy showed a polipoid mass obstructing upper right segmental bronchus. Biopsy was performed and revealed low-grade mucoepidermoid carcinoma. Subsequently, the patient was submitted to a bilobectomy. Nowadays, the patient has been met well in follow-up of 14 months in our department.

ABSTRACT

O carcinoma mucoepidermóide é um tipo de neoplasia encontrada geralmente nas glândulas salivares, mamas, pâncreas e trato digestivo. O acometimento primário da glândula tireóide é muito raro, apenas 33 casos na literatura. Embora a maioria dos casos de carcinoma mucoepidermóide de tireóide (CMET) tenha evolução favorável, o caso clínico relatado descreve uma paciente com tumor agressivo e, a partir deste relato, é apresentada revisão da literatura quanto aos aspectos clínico, histopatológico, imunohistoquímico e origem histogenética dessa neoplasia.


The mucoepidermoid carcinoma is a neoplasia that usually occurs at salivary glands, breast, pancreas and gastrointestinal tract. The primary occurrence on thyroid gland is rare and only 33 cases were previously published. Although the majority of cases of mucoepidermoid carcinoma of the thyroid (MECT) show a benign evolution, this paper describes a patient with an aggressive tumor. A literature review over clinical, histopathological, imunohistochemical features and histogenetic origin was discussed.

Os Tumores das Glândulas Salivares Maiores estão localizados nas Glândulas Parótida, Submandibular e Sublingual e correspondem a aproximadamente 3 a 5 porcento de todas as neoplasias do segmento cabeça e pescoço. O objetivo do presente trabalho é demonstrar o comportamento dos Tumores Benignos e Malignos das Glândulas Salivares Maiores de pacientes atendidos e tratados no Serviço de Cirurgia de Cabeça e Pescoço do Centro de Oncologia (CEON) do Hospital Universitário Oswaldo Cruz (HUOC) - Universidade de Pernambuco (UPE), no período de Janeiro de 1975 a Janeiro de 2003 (28 anos). Em um total de 153 casos (100 porcento), o sexo feminino foi mais prevalente (61,5 porcento), bem como a 4ª e 5ª décadas de vida. Os tipos histológicos benigno e maligno predominantes foram o adenoma pleomórfico (58,2 porcento) e o carcinoma mucoepidermóide (7,8 porcento), respectivamente. A complicação cirúrgica pós-operatória mais freqüente foi a parestesia do ramo mandibular facial (41,9 porcento), seguida da síndrome de Frey (20,8 porcento). Através de um estudo retrospectivo, foram analisados aspectos etiológicos, clínicos, histopatológicos, diagnóstico diferencial, tratamento e prognóstico, e compararam-se os referidos indicadores com os apresentados na revisão da bibliografia consultada. Demonstram, ainda, sua experiência com a referida patologia, e suas conclusões do material estudado
